*I acknowledge that I have had the full opportunity to ask any questions about getting a tattoo, and they have been answered to my satisfaction. If I take blood-thinning medication, I have informed my tattooist. I confirm that I am not pregnant, nursing, or under the influence of alcohol or drugs.

*I do not have medical or skin conditions such as acne, keloid scarring, eczema, psoriasis, rash, infection, lesion, freckles, moles, or sunburn in the tattoo area that may interfere with the process. If I have any infection, rash, or lesion anywhere on my body, I will inform my tattooist.

*I understand that it is not possible for the shop's representatives or employees to determine if I may have an allergic reaction to the pigments or process used in my tattoo. I accept the risk that such a reaction may occur.

*I acknowledge that infection is possible, especially if I do not properly care for my tattoo. I have received aftercare instructions and agree to follow them. Any touch-up work needed due to my negligence or failure to follow aftercare will be at my own expense.

*I understand that color and design variations may exist between the tattoo I select and the final result. If my skin is darker, I acknowledge that colors may not appear as bright as on lighter skin.

*I understand that skin treatments, laser hair removal, plastic surgery, or other skin-altering procedures may cause changes to my tattoo.

*I acknowledge that a tattoo is a permanent change to my appearance, and no guarantees have been made about its ability to be removed or altered later. To my knowledge, I do not have any physical, mental, or medical condition that may affect my well-being as a result of getting a tattoo.

*I confirm that I am over 18 and have truthfully represented my age. I understand that getting a tattoo is my personal choice, and I consent to the procedure and any necessary actions by the tattoo shop's employees to complete it.
